Suddenlink Subscriber Agreement

i. Dispute settlement. Before filing a legal action in an arbitration, you must first inform us of your dispute and give us the opportunity to resolve it without the need for arbitration. You must write us a letter in which you briefly explain the dispute and outline the relief you have asked for. Provide as much information as possible, including the corresponding data and certain amounts of money. Please also provide the name of the account holder, account number, service address and phone number where you can be reached during business hours. For your convenience, you can download a dispute resolution form from our website at www.suddenlink.com/sites/default/files/Notice-Of-Dispute.pdf. Once you have written or filled out the letter, email us to Altice Shared Services, 200 Jericho Quadrangle, Jericho, NY 11753, Attn: Customer Litigation. 3. Fees for third parties. With respect to the customer`s use of services and equipment, the customer may have access, subscribe, use and/or purchase products, services, software or applications made available to the customer by third parties („third parties“). The customer recognizes that the customer may be subject to a subscription, purchase or use fee for these third-party products, services, software or applications.

All of these fees, including additional fees and applicable taxes, are paid by the customer to a third party and are not the responsibility of Suddenlink. Credits or billing adjustments for products, services, software or applications billed by a third party are subject to billing practices indicated by that third-party provider. The termination of a service or subscription offered for a separate fee charged directly by a third party is carried out in accordance with the terms of use or a similar agreement between the customer and the third party.
